WALDORF WILDCATS 2008 ONLINE REGISTRATION FORM

Registration will open until June 30, 2008.  Registration for the upcoming season will guarantee your son or daughter a roster spot on a Wildcat football or cheerleading roster, however it does not guarantee a spot on a specific team or squad.  Be advised that once your child is registered online it is mandatory for you and your child to attend our last registration day at John Hanson Middle School, which will occur in July.  All registered players must attend on this date to be fitted, weighed and all documents signed by a parent or legal guardian.  If you register and do not show you will forfeit your spot on the team or squad and you will forfeit all money paid.

Football News

Please register your child in accordance with the new 2008 County weight chart.  Falsification of weight will void the registration and NO refund will be issued.

Cheerleading News

Age and grade verification: Charles County Public Facilities has initiated a new age/grade verification policy for its Recreation Cheerleading Squads.  Each parent or legal guardian must register his or her child at the correct age/grade listed for 2008/2009 school year. Parents must also submit his or her child's 2008 3rd or 4th quarter report card at the June registration.  Due to continued league growth and attendance at county and region-wide competitions, adherence to the team age/grade guidelines is paramount. Be sure to register your child at the appropriate age/grade level.  Falsification of age or grade will void the registration and NO refund will be issued.

To cheer, a cheerleader must be between the ages of 5 and 14.  A cheerleader must be 5 as of August 1, 2008.  A 9th grade cheerleader cannot cheer on an all-star or high school team.

As stated above, you must register your child at the correct age/grade level. However, working within the 20% up/down maximum allowance, you may complete the "Special Squad Request" area on the on-line form. This area allows you to submit a special request for team placement for a specific reason. The Board will consider special requests once ALL registrations are finalized.  Special squad placements are NOT guaranteed. The Board will consider each request individually, while working within the new County age/grade guidelines.

Walk Up Registration

The cost of registration will be $155 per child.  You and your child need to be present so that you child may be weighed-in, fitted for their equipment/uniform, and to sign required county forms.

As parent or legal guardian of child listed on this registration form, I hereby indemnify Waldorf Youth Sports, Inc and hold said organization blameless for any injury(ies) incurred during any sponsored activity of Waldorf Youth Sports, Inc., including practice.  I also give my permission for any and all medical attention necessary to be administered to my child in the event of an accident, injury, sickness, etc. under the direction of Waldorf Youth Sports, Inc and its representatives until such time as I can be contacted.  I also understand that Waldorf Youth Sports, Inc recommends physicals for children participating in the football/cheerleading program and requests that a parent or other designated guardian attend all practices and games.

 

CCYFL REGISTRATION FORM WAIVER:


My Child/I (Registered Child & Parent/Legal Guardian of the registered child), agree(s) to participate in the above program knowing that safety precautions will be taken but realize that the Charles County Department of Public Facilities and Charles County Youth Football League (CCYFL) do not have accident insurance for the participants of this program.


I (Parent/Legal Guardian), do hereby release and hold harmless Charles County, Maryland, CCYFL, Waldorf Youth Sports, Inc. its officials, employees, instructors and volunteers from any and all liabilities arising from any injuries that might occur during the supervised program.

I also authorize the Charles County Department of Public Facilities, the Charles County Youth Football League and Waldorf Youth Sports, Inc. to take photographs of me/my child for promotional and/or education purposes.

It is hereby stated and declared by me that the released information stated above is freely, willing, and voluntarily made. Furthermore, I understand that if any of the above registration information is found to be intentionally falsified, that my child will be expelled from the program.

1. Authority to Register and/or to Act as Agent. You represent and warrant to Waldorf Youth Sports, Inc. that you have full legal authority to complete this event registration on www.waldorfwildcats.com, including full authority to make use of the credit or debit card to which registration fees will be charged. In addition, if you are registering third parties, you represent and warrant that you have been duly authorized to act as agent on behalf of such parties in performing this event registration. By proceeding with this event registration, you agree that the terms of this Registration Agreement shall apply equally to you and to any third parties for whom you are acting as agent. Compliance with Children's Online Privacy Protection Act (COPPA). You represent and warrant that, in compliance with COPPA, you are over thirteen (13) years of age, and that if you are registering a child under fourteen (14) years of age you are the parent of such child, and do hereby consent to the collection of such child's personal information by Waldorf Youth Sports, Inc.

2. Limitation of Liability; Disclaimer of Warranties. Waldorf Youth Sports, Inc SHALL NOT BE LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL OR CONSEQUENTIAL DAMAGES, RESULTING FROM (A) THE USE OR THE INABILITY TO USE THE WALDORF YOUTH SPORTS, INC WEBSITE OR (B) FOR THE COST OF PROCUREMENT OF SUBSTITUTE GOODS AND SERVICES OR (C) RESULTING FROM ANY GOODS OR SERVICES PURCHASED OR OBTAINED OR TRANSACTIONS ENTERED INTO THROUGH THE WALDORF YOUTH SPORTS, INC WEBSITE OR (D) RESULTING FROM UNAUTHORIZED ACCESS TO OR ALTERATION OF YOUR TRANSMISSIONS OR DATA, INCLUDING BUT NOT LIMITED TO, DAMAGES FOR LOSS OF PROFITS, USE, DATA OR OTHER INTANGIBLE, EVEN IF WALDORF YOUTH SPORTS, INC HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. YOU EXPRESSLY AGREE THAT USE OF THE WALDORF YOUTH SPORTS, INC WEBSITE IS AT YOUR SOLE RISK. THE WALDORF YOUTH SPORTS, INC WEBSITE IS PROVIDED ON AN "AS IS" AND "AS AVAILABLE" BASIS. 4. Applicable Law; Consent to Jurisdiction. The Waldorf Youth Sports, Inc sites (excluding linked sites) are controlled by Waldorf Youth Sports, Inc from its offices within the State of Maryland, United States of America. By completing this event registration, both you and Waldorf Youth Sports, Inc agree that the statutes and laws of the State of Maryland, without regard to the conflict of laws principles thereof, will apply to all matters relating to this event registration, this Liability Waiver, or other use of the Waldorf Youth Sports, Inc sites. You agree that exclusive jurisdiction for any dispute with Waldorf Youth Sports, Inc resides in the courts of the State of Maryland and you further agree and expressly consent to the exercise of personal jurisdiction in the courts of the State of Maryland in connection with any dispute including any claim involving Waldorf Youth Sports, Inc or its affiliates, subsidiaries, employees, contractors, officers, directors, telecommunication providers and content providers.

5. Severability. If any provision of this Liability Waiver shall be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this Liability Waiver and shall not affect the validity and enforceability of any remaining provisions.

6. Waldorf Youth Sports, Inc shall only use any Personal Identifying Information for the purpose of order processing, fulfillment, customer service, and renewal purposes. Waldorf Youth Sports, Inc shall not use any Personal Identifying Information for marketing or solicitation purposes or for any other purpose not explicitly permitted in this Agreement. "Personal Identifying Information" shall include any information about users of Waldorf Youth Sports, Inc's website.
